ABCs
Grade 3-4 encephalopathy: cerebral edema, or substance ingestion, or shock
Mental status exam and send lab studies ASAP for appropriate triage to ICU or transplant center
Rapid evaluation for etiology
If acetaminophen (APAP) toxicity is suspected, do NOT delay treatment.
