History and Physical (assess for the following)

Measure and record vital signs regularly throughout dialysis.
Hypotension: make sure dialysis access is not clotted & volume of blood in the dialysis machine is not lost.
If dialysis flow not adequate, make sure central line is not clotted or kinked.
