First Things First (assess & treat for the following)

Get a decubitus CXR to evaluate if fluid present.
- >1 cm more likely to be significant
- Determine if fluid layers (nonloculated).
Perform diagnostic/therapeutic thoracentesis.
- Determine if fluid exudative or transudative.
- Treat as appropriate.
