DVT Prophylaxis

First Things First (assess & treat for the following)

• Assess risk factors for VTE (see “History and Physical”).
• Assess for contraindications to pharmacologic thromboprophylaxis
  • Active uncontrolled bleeding
  • High risk of spinal hematoma (fracture, spinal surgery)
  • High risk of intracranial hemorrhage (trauma, recent stroke, brain surgery)
  • Severe thrombocytopenia
  • History of heparin-induced thrombocytopenia (no UFH or LMWH)
