Neuraxial blockade should be avoided in the presence of prophylactic or therapeutic anticoagulation because of the increased risk of epidural hematoma formation. Massachusetts General Hospital guidelines regarding neuraxial anesthesia in anticoagulated patients are listed in Table 17.3.
