First Things First (assess & treat for the following)

  • Get a decubitus CXR to evaluate if fluid present.
    • >1 cm is more likely to be significant.
    • Determine if fluid layers (nonloculated).
  • Perform diagnostic/therapeutic thoracentesis.
    • Determine if fluid exudative or transudative.
    • Treat as appropriate.
