First Things First (assess & treat for the following)
- Does pt have pleural effusion & clinical picture consistent w/ pneumonia (eg, parapneumonic effusion)? Start empiric antibiotics after blood cultures are done.
- Diagnostic thoracentesis for moderate to large effusions using ultrasound or CT guidance unless contraindicated (eg, severe coagulopathy)
